You can keep collecting extra details, like birthdays or preferences, with Shopify's Customer Accounts. Your form just moves from the registration page into the customer's account.
What changes
Shopify now hosts the sign-in page, where customers sign in with their email and a one-time code. Apps can't add fields to it. Instead, use an Account/Checkout form. It shows in the customer's account or on the Thank you page, and saves answers to the customer's metafields.
Where the form can appear
Extension | Where it shows |
Custom profile block | A card on the Profile page, where customers add or edit their answers. |
Custom profile page | Its own page in the customer account. |
Post-checkout form | The Thank you page, for signed-in customers only. |

Remind customers to fill it in
Add a Call to action banner to the Orders page, with a button that takes customers to the form, or a Notification card to the Profile page, which shows a short message above the form. Both disappear once the form is submitted. See Customer account extensions for each one's settings.

Set it up
Go to Forms > Create form and choose Account/Checkout.
Add your custom fields and save. To reuse a field from your old form, open its Data column settings and choose Select an existing data column.
On the Install form page, open Shopify's checkout and accounts editor, add an extension and paste your form's ID into its Form ID setting.
Save in the editor, and publish if you're working on a draft.
Tag customers who fill it in
On the Pro and Advanced plans, a form rule with the add tag to customer action can tag customers when they submit, for example based on an answer. Use the tag in segments, Flow or marketing. See Customer auto tagging.
What to know
Account/Checkout forms are on every plan. They save custom fields only, they do not save Shopify's Standard name, email or address fields, and only reach signed-in customers. They don't support account approval; for that, see Keep approving customers on Shopify's Customer Accounts.
If your store uses the bundled JSON metafield storage method, this form type is locked. Switching under App settings > Metafields > Metafield storage strategy migrates your customers' metafields, which can affect theme code or other apps, so check with your developer first.
Use a storefront form instead
To reach people who don't have an account yet, like newsletter or event sign-ups, use a Storefront form with Default under Account options. Add it to a page with an app block or a shortcode, and link to that page from your navigation, since the Account icon opens Shopify's sign-in page.
When someone submits, Customer Fields creates the customer right away, with no review, password or email. Signed-in customers update their own details. If the email already belongs to a customer, Customer Fields sends an Account verification email and saves the answers once it's confirmed, or shows an "already in use" error.
To follow up, use the Customer submitted form trigger in Shopify Flow, or turn on Send an email when a new customer signs up for an account under App settings > Email notifications.
If you need to review sign-ups before they become customers, choose Require account approval instead. See Keep approving customers on Shopify's Customer Accounts.
